Trinidad And Tobago VISA Application form (for US & Israeli citizens only not military or security service personnel.)

All applicants Must fill in and sign a declaration form; (All Questions must be answered with a simple tic in the appropriate box).Forms must be submitted 12 months before the date of intended travel.

Questions :
1. Do you believe that any nation has the right to invade kill and cause the murder of 10s of thousands? [] yes……….. [] No.

2. Are you or your family, members of any security military or secret “state” services? [] Yes……. [] No

3. Are you or members of your family xenophobes? []yes……..[]no

4. Have you denounce state terrorism? [] yes………….[]No.

5. Have you ever been involve in any form of terrorism outside of the US? [] Yes……..[]No.

If yes state date and country…………….

6. Have you ever served US interest anywhere outside of the US []Yes……….[] No

State what were your functions and your employer’s name and address.

7. Are you prepared to undergo a polygraph? [] Yes……..[] No

15. Have you ever been to the School of Americas and in what year did you graduate and where was you first assignment?

16. Who were your colleagues and instructors? Please list their names nationality and address at the blank side of this form and certify it with a Notary public.

Note that questions improperly answered shall be considered an act to conceal terrorist activity and can lead to detention without trial at golden grove prison.

All successful applicants/travelers will be finger printed and have their photos taken for their own personal security at their own expense.

Walk with your toilet paper as TT will not undergo the costly finger print electronic upgrade, note that all 10 fingers and hands will be printed.
